The calcaneus, or heel bone, is the large bone at the back of the foot. The calcaneus provides critical support for the foot and facilitates the walking motion. The majority of calcaneal fractures occur in high velocity car accidents or truck accidents.

Calcaneal Fractures

The subtalar joint, located between the calcaneus and the talus, is responsible for the inward and outward rotation of the foot. When the calcaneus is fractured, this rotation may be limited or lost completely.

It takes a great amount of force to fracture the calcaneus, but surprisingly most calcaneal fractures are closed fractures. Due to the amount of force, the damage can be extensive, requiring open reduction and internal fixation.

There are many problems associated with calcaneus fractures. One is the widening and possible deformity of the bone. Another problem can be damage to the cartilage of the subtalar joint that leads to arthritis. Calcaneus fractures may also cause injuries to the heel pad, including nerve and tendon damage.

The goals of surgery are to realign the fragments, restore the correct structure, and minimize loss of blood supply to the bone which will decrease the chance of avascular necrosis. It is recommended that no weight be applied to the foot for about 3 months.
