The patellar tendon extends from the top of the tibia to the base of the patella. It plays a vital part in the ability to extend the leg. If the patellar tendon ruptures, the patella becomes displaced by the quadriceps muscle. The result is intense pain and the inability to straighten the leg.
Patellar Tendon Injury
It takes a high intensity force to rupture the patellar tendon. Ruptures happen most often in high impact car crashes or slip and fall accidents. Symptoms of a patellar tendon rupture are:
- intense pain
- severe bruising
- deformity in the leg
- inability to extend the leg
- inability to bear weight
Surgery is required for a patellar tendon rupture. It does not usually respond well to repair, making patellar tendon replacement surgery the best option.
A graft is used from the posterior femoral muscle. The patellar tendon is trimmed, but left attached to the tibia. A hole is drilled through the base of the patella. One end of the graft is sutured to the base of the patellar tendon. The graft tissue is then threaded through the drilled hole, looped back down, and sutured next to the other end; pulling the patella back into position.
Complications with this surgery include:
- the patella may fracture when it is drilled or when the graft is sutured down
- the graft may be too short
- the patellar tendon may pull out from its position in the tibia
The knee should be in a cast or brace for 6 weeks after surgery, then 6 weeks in a hinged brace. Recovery can be complete with adherence to a physical therapy program.
