A separated shoulder is not a shoulder joint injury. It is an injury to the acromioclavicular (AC) joint. The AC joint is where the clavicle meets the acromion, the highest point of the shoulder. Most separated shoulders are soft tissue injuries, although they may include a fracture.
A shoulder separation occurs most frequently in slip and fall cases, but they can happen in motor vehicle accidents by being forced against the door or bracing for impact against the dash.
There are three grades of shoulder separation:
- In a mild shoulder separation, the injury is a sprain of the AC ligament. The collarbone is not moved and looks normal on x-rays.
- In a more serious shoulder separation, the AC ligament is torn and the coracoclavicular (CC) ligament is sprained or torn. The collarbone is out of alignment, though may be evident only on x-rays.
- In a severe shoulder separation, the AC and CC ligaments are completely torn. The AC joint is visibly out of alignment.
Separated Shoulder Treatment
All three grades of separation have to be immobilized to allow the swelling to subside prior to treatment. For grades 1 and 2, immobilization is usually all that is required. The ligaments will heal in about three weeks. For a grade 3 injury, surgery may be recommended to repair the torn ligaments. No matter the degree of injury, it is vital to start moving your shoulder when the pain subsides to avoid a frozen or stiff shoulder.
