The forearm consists of the ulna and radius. Joint capsules connect them at their proximal and distal ends. Fractures are classified as proximal, middle, or distal shaft; these fractures often times impact the elbow and wrist joints. The most common causes of ulnar radial fractures are slip and fall accidents, bicycle accidents, and motor vehicle accidents.
Treatment of Ulnar Radial Fractures
Standard X-rays, with multiple angles, will be taken of the broken arm to reveal the alignment and fragment position. X-rays will be repeated after the bones are set, to verify that the reduction is complete, and then at weekly intervals to make sure the position is still acceptable. With a complicated fracture, the physician may request CT or MRI scans before and after the fracture is set to help determine if surgery is necessary. If there is any suspicion of nerve or vessel damage, nerve conduction studies and vascular studies need to be ordered.
Generally, in adults, a ulnar radial fracture will require surgery. This type of double fracture tends to be unstable and requires internal fixation for proper alignment and healing. Surgery requires two incisions because both bones will need a metal plate and screws — possibly a rod along side the plate. A rod is used in relatively few cases because it interferes with rotational stability of the arm.
Ulnar Radial Fracture Prognosis
The critical time in the course of recovery is between 2 and 3 weeks. During this time, the swelling will decrease and the fracture may displace. At the 3 weeks point after injury, the healing will be far enough along as to limit treatment options if the fracture has become displaced.
Cast time recommendations vary from 3 to 6 weeks. In adults, the risk of stiffness of the joint has to be taken in consideration. If any issues arise, the cast may be replaced by a removable splint. After the cast is removed and the doctor has determined that enough healing has taken place, physical therapy will be necessary for recovery of strength and range of motion.
